Can postpartum dysmenorrhea really be improved?

Dysmenorrhea is one of the most common symptoms among gynecological patients, plaguing many women. Some people say that postpartum dysmenorrhea can be improved. Is this true? In this regard, Xu Li, deputy chief physician of the Department of Gynecology at the Second Hospital of Traditional Chinese Medicine in Guangdong Province, said that since the causes of dysmenorrhea are different for each person, whether postpartum dysmenorrhea improves varies from person to person. To people.

The improvement of dysmenorrhea has nothing to do with confinement

Dysmenorrhea is divided into two types: primary dysmenorrhea and secondary dysmenorrhea. Primary dysmenorrhea has no organic disease, while secondary dysmenorrhea is caused by organic pelvic disease, such as endometriosis, pelvic inflammatory disease, cervical stenosis, etc. Primary dysmenorrhea generally has no abnormal signs, so it is difficult to say what causes it. However, it can be affected by mental, neurological, genetic, immune and other factors, and it mostly occurs in teenagers. Symptoms will gradually disappear with age, complete uterine development, and assisted reproduction after marriage.

For secondary dysmenorrhea, some people\’s dysmenorrhea will suddenly get better after giving birth. Therefore, some people believe that confinement can \”sitting\” dysmenorrhea. Xu Li said that was not the case. Confinement is just an appropriate adjustment for the weak body after childbirth. Essentially, it does not treat organic pelvic disease. Therefore, it is impossible to bring good curative effect to dysmenorrhea.

The improvement of postpartum dysmenorrhea varies from person to person

It is not impossible for postpartum dysmenorrhea to get better on its own, but it is more a function of the reproductive process and varies from person to person. Xu Li explained that for some pelvic organic diseases that cause dysmenorrhea, such as cervical stenosis, endometriosis, etc., some women will get better during the childbirth process. For example, if the cervix is narrowed and menstrual blood cannot flow out easily, causing dysmenorrhea, the cervix will expand during delivery. Even if the cervix recovers after delivery, it will not be as narrow as before, which can improve the dysmenorrhea caused by cervical stenosis. But Xu Li emphasized that this situation must be effective for women who have had a natural birth or experienced a natural birth process. Postpartum dysmenorrhea is not likely to improve in women who have had a direct caesarean section because they have not experienced the stage of cervical dilation.

In addition, endometriosis has a greater impact on fertility. Endometriosis is a common gynecological disease. During menstruation, patients with endometriosis are prone to reflux of menstrual blood, which means that menstrual blood flows backward through the fallopian tube and into the abdominal cavity, causing problems such as dysmenorrhea. Xu Li said that for endometriosis, pregnancy is the best treatment. Since there is no menstruation for 9 months of pregnancy, the ectopic endometrium may shrink, so that some patients with endometriosis can achieve cure, and even if they cannot be cured, the symptoms will be relieved. Xu Li added that for this type of patients, breastfeeding is also encouraged, because breastfeeding will delay menstruation. The longer the period is without menstruation, the better the effect of treating endometriosis. In other words, the better the resulting dysmenorrhea problem is improved.

Pay attention to hygiene during confinement period

Although the improvement of dysmenorrhea has nothing to do with confinement, if you do not pay attention to hygiene during confinement,If it happens, it will cause or aggravate dysmenorrhea. This is why some women have questions about \”postpartum dysmenorrhea\” or \”it doesn\’t get better, but gets worse.\” Xu Li said that the traditional concept is that you cannot take a bath or wash your hair during the confinement period for fear of being caught by the wind and leaving the root of the disease. In fact, this understanding is unreasonable. During confinement, women have more genital secretions. Wash the vulva with warm water every day, replace the perineal pad frequently, and keep the perineum clean and dry. Xu Li also emphasized that postpartum sex should not be carried out too early, and sex is absolutely prohibited within 42 days after delivery, otherwise it will easily cause pelvic inflammatory disease and then cause dysmenorrhea.

In addition, sudden postpartum dysmenorrhea may also be due to adenomyosis. The highest incidence age of adenomyosis is 30-40 years old, secondary to postpartum, induced abortion, dilation and curettage, etc. Therefore, if a woman with no history of dysmenorrhea suddenly develops progressive dysmenorrhea after childbirth, accompanied by increased menstrual flow, she should be alert and go to the hospital for examination in time.
